Delta Dental PPOSM

- Delta Dental Premier network dentists accept Delta Dental’s contracted fee as payment in full.
- Freedom to choose any dentist – no pre-authorization or referral required.
- Widest choice of dentists with the nation’s largest dental network (more than 240,100 locations).
- Premier dentists offer balance-billing protection and greater potential savings than non-Delta Dental dentists.
- Low network turnover – less than 2%.
DeltaCare® USA
- Pre-paid program similar to a dental HMO plan.
- Set copayments, no annual deductibles, and no maximums.
- In most states, enrollees select a primary care dentist from whom they receive treatment.3
- Comprehensive coverage – more than 250 procedures covered.
- Stable network of dentists.
- Low network turnover – less than 2%.
