My Dentist

With the DeltaCare® USA plan, you select your contract dentist when you enroll in the plan. You must receive treatment from your assigned DeltaCare USA contract dentist who will coordinate your dental care and will refer you to a specialist for specialist services. Specialist services may require preauthorization. Except for dental emergencies and preauthorized specialty services, treatment provided by a dentist other than your assigned contract dentist will not be covered.

Changing My Dentist

If you wish to change your dentist, you can submit a customer service request form or call Customer Service at 800-422-4234.

Changes made by the 21st day of the month will be effective the first of the following month.

Note that if you have treatment in progress, such as dentures or crowns that have been prepared, you must have the service completed before you change to another dentist.
