How do the plans work?
- Coverage is effective for 12 months and renewal is required to continue coverage.
- To enroll yourself or other eligible dependents simply pay the annual premium and a one time enrollment fee.
- During enrollment, you choose the DeltaCare® USA dentist from whom you will receive dental care. You and your eligible dependents must receive dental care from your assigned DeltaCare USA contract dentist.
- You will pay an office visit fee and the listed copayments for the covered procedures you receive.
