Who is eligible to enroll?
- You, your spouse or domestic partner and children up to age 26
- A child over the age of 26 may remain eligible if that child is incapable of self-sustaining employment because of a physically or mentally disabling injury, illness or condition and is chiefly dependent on you for support and maintenance.
Note that all enrollees must select and receive care from the same contract dentist.
