You, your spouse or Domestic Partner and each of your unmarried dependent children from birth to age 19, or 23 if they are Full-Time Students, are eligible to join the DeltaCare USA Individual/Family Dental HMO Program. Dependents in military service are not eligible.
spouse (unless legally separated or divorced) or Domestic Partner (until such partnership is terminated by either or both parties);
unmarried children from birth up to age 19;
dependent children until they turn 23 if they are wholly dependent on you for support and are Full-Time Students in an accredited school.
Children include natural children, stepchildren, adopted children, foster children and children of a Domestic Partner if they are dependent on you for support. Newborn children are covered from and after the moment of birth or placement. Notification of birth must be received within 31 days after the date of birth or placement to continue beyond 31 days. Foster children and adopted children are eligible from and after the moment they are placed in your physical custody.
An unmarried child over the age of 19 may continue to be eligible if that child is incapable of self-support because of a physical disability or mental incapacity and is chiefly dependent on you for support and maintenance.
Your spouse or Domestic Partner and children become eligible on the date you enroll for coverage, immediately upon becoming dependents, or at any time subject to a change in legal custody or lawful order to provide Benefits. To be eligible, you must live or work in the DeltaCare USA service area.
