For your convenience, Delta Dental has made it possible to choose from two payment options. Annual Premium may be charged to your MasterCard or Visa account, or you may pay by personal check or money order. Be sure to indicate which payment option you have chosen on the Enrollment and Payment Authorization Form.
Credit Card Payment Option
If you choose the Credit Card Payment option, your annual Premium and the $15.00 onetime enrollment fee will be charged to your MasterCard or Visa account.
Check/Money Order Payment Option
If you prefer to pay by personal check or money order, select that option on the Enrollment and Payment Authorization Form and make your check payable to Delta Dental of California.
