General processing tips
Completing the claim form
- Be sure all information is entered on the claim form
- Complete all sections of the standard approved forms
- Document exceptional or unusual circumstances
- Use the comments/notes field of an electronic claim or any available space on a paper claim.
Submitting x-rays
- Only a few procedures require the submission of x-rays. Refer to the page on when to send x-rays for more information.
Student information
- To facilitate processing, please provide current student information for dependents age 18 and over on all appropriate claims.
Claims for twins
- You can help Delta Dental process benefits for twins (triplets, etc.) by printing "TWINS" (TRIPLETS, etc.) above the patient's date of birth on the claim. Because Delta Dental relies on the date of birth to identify dependent children, identifying the patient as a twin helps us avoid entering dental services into the wrong patient's records.
Change of office address
- Delta Dental's Dentist Network Administration and Contracting department must be informed in writing of any address changes, the closing of an office or the opening of additional offices. Download and print the necessary form, which can be faxed or mailed to Delta Dental (requires Adobe Acrobat Reader, which can be downloaded for free).
