Claims and paperwork

Do I need to submit a claim after receiving treatment or will my dentist handle it?

One of the advantages of visiting a Delta Dental dentist is that Delta Dental dentists will take care of all claims paperwork for you.

However, if you visit a non-Delta Dental dentist,, you may have to handle this process on your own. Non-Delta Dental dentists typically do not submit claims to Delta Dental on your behalf. Please see next question for instructions on submitting a claim to Delta Dental.

If I visited a non-Delta Dental dentist, what is the process for submitting a claim?

First, see our instructions for downloading the Delta Dental claim form. This requires Adobe Acrobat Reader. If you don't have this application already installed on your computer, click here for a free download.

After you've completed the claim form, make a copy for your records and mail it to the address on the form.

Claims are usually processed within 2 weeks unless additional information is required from you or the dentist.

I wasn't issued an ID card when I enrolled. Why?

If you still would like an ID card, you can print one online

You can print a customized ID card on demand using our online benefits and eligibility system. Just log in by entering your user name and password in the area at the top left of this page.

If you do not have a user name and password, click the link to register.

Just as you no longer must present a ticket at an airline check-in counter to confirm your reservation, enrollees of Delta Dental's fee-for-service programs (Delta Dental Premier and Delta Dental PPO) don't need an ID card to verify their Delta Dental coverage.

When visiting a Delta Dental dentist, simply provide your social security or identification number. The dental office can use that information to verify your eligibility and benefits through Delta Dental's web site or toll-free number. And, you can visit any dentist anywhere in the world and never have to worry about forgetting an ID card for a dental appointment.

I need to change my address. Where can I submit this information?

Delta Dental normally uses the address submitted by your dentist on your most recent claim form, so make sure that your dentist has your current address. Delta Dental may also obtain your address from your employer or program sponsor.

Delta Dental mails your Notice of Payment (a statement explaining what services the dentist provided, the amounts Delta Dental will pay and the amount you are responsible for paying) to the primary enrollee, who is the person with the dental coverage.

I've recently married. How do I add my new spouse to my plan?

Delta Dental receives the information about covered family members from your employer, union or other group sponsor. Please follow the procedures at your company to add, delete or change information about covered family members.
