Resolving Problems
If you have questions about your child's dental care, talk to your child's Main Dentist. He or she may be able to answer your questions. If, after talking with your dentist, you are still not happy, there is a formal Complaint, and Appeal process to address your concerns.
Complaints
If you are not happy and would like to file a complaint, you can fill out a Complaint Form or contact us for help.
Appeals
You can file an appeal if you are not happy with the complaint. You cannot appeal a decision about the annual maximum benefits or member eligibility. Please use our Appeal Form.
Filing with the Texas Department of Insurance
You can file a complaint with the Texas Department of Insurance at any time. You do not have to contact Delta Dental first. Contact the Texas Department of Insurance for a form and instructions on filing a complaint.

Reporting Waste, Abuse and Fraud
If you think a doctor, dentist, or other health care providers, or a person getting benefits is doing something wrong, let us know. We'll investigate. Waste, abuse and fraud is not only against the law, it increases dental care costs for all.
What is Waste, Abuse and Fraud
Here are some examples:
- Getting paid for services that weren't given or necessary
- Not telling the truth about a medical condition to get medical treatment
- Letting someone else use their CHIP ID
- Using someone else's CHIP ID
- Not telling the truth about the amount of money or resources he or she has to get benefits
How to Report
You can report online, by phone or by mail.
- Telephone: Call the OIG Hotline at 1-800-436-6184.
- Online: Visit The Office of Inspector General and complete the online form.
- Mail: You can report directly to us, please fill out our Waste Fraud and Abuse Form.
What to Report
Gather as much information as possible. When reporting about a provider (a doctor, dentist, counselor, etc.) include:
- Name, address, and phone number of provider
- Name and address of the facility
- CHIP number of the provider and facility, if you have it
- Type of provider (doctor, dentist, therapist, etc.)
- Names and phone numbers of other witnesses who can help in the investigation
- Dates of events
- Summary of what happened
