No statement in the application will void this Contract or be used in any legal proceeding unless the application, or an exact copy, is included in, or attached to, this Contract. This Disclosure Form/Contract, and any attached schedules, appendices, endorsements and riders, constitute the entire agreement governing the Program. No amendment is valid unless approved by an executive officer of Delta Dental and attached to this Contract. No agent or broker has authority to amend this Contract or waive any of its provisions. |
