| This Contract, and any attached schedules, appendices, endorsements and riders, constitute the entire agreement governing the Program. No amendment is valid unless approved by an executive officer of Delta Dental and attached to this Policy. No agent or broker has authority to amend this Contract or waive any of its provisions. |
