All Costco Executive members are eligible for this Delta Dental plan. There is no pre-screening and no proof of previous coverage required.

If you enroll and pay your premium by the 21st of the month, you can begin using your dental benefits plan by the 1st of the next month.

All you have to do is to complete the plan enrollment form, confirm that you understand the plan details and submit payment.

Renew coverage

You can renew coverage online anytime, as long as it's at least 90 days before the coverage termination date.

If your coverage has been terminated for more than 15 days, please contact Customer Service to get reinstated or re-enroll in the plan.
