You can file an appeal if you are not happy with the outcome of your complaint. However, you can not appeal a decision related to annual maximum benefits or member eligibility. If you have questions, you may contact our Member Call Center at 866-561-5892.

If you prefer, you can file a complaint with the Texas Department of Insurance (TDI) at anytime.
You do not have to contact Delta Dental first. You can contact the TDI at the address and phone number below for claim forms and instructions on filing a complaint. You can also get claim forms and information on the TDI’s web site.
