Transportation benefits for CDPHP members
CDPHP Medicaid Select Plan and Medicaid HARP members may be eligible for transportation to and from appointments for Medicaid-covered services, including dental appointments. Learn more about transportation benefits provided by the New York State Medicaid program.
CDPHP medical dental
Dental problems caused by accidents or by birth defects (like a cleft palate) are considered medical dental issues. For medical dental issues, please contact CDPHP at 1-800-388-2994.
For routine services that don’t involve medical dental issues, such as dental checkups, x-rays, and fillings, you don’t need to contact CDPHP. Simply visit your network dentist for treatment.
Emergency/after-hours support
If you’re experiencing a medical or dental emergency, please call 911.
If you’re in pain and need dental assistance outside of regular business hours, please call your network dentist. If you can’t reach your dentist, please call CDPHP at
1-800-388-2994.
