  • Business name
  • ZIP code
  • County
  • Standard Industrial Classification (SIC) code
  • Number of eligible employees
  • Employer’s dental contribution
  • Whether the client has prior comprehensive coverage

To get credit for your sales, you must be appointed with Delta Dental. Once you’re registered, you can apply for appointment. If you need help, reach out to our Producer Services team.
