Visit your DeltaCare USA dentist. When you enroll, you’ll be assigned a general dentist in the DeltaCare USA network who will provide your primary care. You must visit your selected DeltaCare USA primary care dentist to receive benefits under your plan. If you’d like to choose a different DeltaCare USA dentist, you can make a change request at any time — just log in to Online Services on this page or call Customer Service at 800-422-4234 to select a different dentist. The DeltaCare USA network consists of private practice dental facilities that have been carefully screened for quality. If you need to see a specialist, your DeltaCare USA dentist will refer you to one.

In the following states, you can maximize your savings when you visit a DeltaCare USA dentist, although you may visit any licensed dentist and receive out-of-network coverage: AK, CT, LA, ME, MS, MT, NC, ND, NH, OK, SD, VT. Refer to your plan booklet for details about your out-of-network benefits.
