San Francisco Healthy Kids is a health insurance program offered by San Francisco Health Plan (SFHP), providing complete medical, dental, and vision coverage to low and middle income San Francisco children aged 0-18.
Delta Dental contracts with the SFHP to provide dental services to children of low and middle income families in San Francisco. To be eligible for the program, children must be uninsured, aged 0-18, and residents of San Francisco who are not eligible for Medi-Cal or the Healthy Families Program. New! Effective July 1, 2009
San Francisco Healthy Kids (SFHK) will only cover members from 0-18 years of age, therefore, eliminating the portion of the program that covers adults from ages 19-24. SFHK will also be instituting a new yearly benefit maximum of $1500.00 per member also effective July 1, 2009. The benefit maximum will be based on a 12-month period effective July 1, 2009 through June 30, 2010. All services performed on a SFHK member with dates of service on or after July 1, 2009 will apply to the $1500.00 maximum. Please keep in mind that all claims must be submitted within 6 months from the date the services were performed to be reimbursed by Delta Dental.
