Delta Dental — Philips Oral Healthcare BrushSmartTM Program User Agreement

How the BrushSmart Program works:

Voluntary participation. Your participation is completely voluntary. You are free to choose not to participate, withdraw your consent or to discontinue to participate in the BrushSmart Program. Those actions will not result in any penalty or loss of benefits on your part.

Data collection and privacy policy. By participating in this Program you authorize Delta Dental to disclose to Philips that you have Delta Dental coverage and that these companies may transmit, analyze and manage your data. Philips will securely share the following data with Delta Dental via a secure FTP server using standard encryption and security measures to ensure your privacy and confidentiality: date, frequency of brushing, time stamp (AM/PM), duration, brushing mode and type of brush head used.

Delta Dental will provide aggregate-level de-identified data to the Employer Group on brushing habits, satisfaction and the like. Delta Dental’s Privacy Policy (available at: https://www.deltadentalins.com/about/privacy/) applies to all claims and enrollment information collected and maintained by Delta Dental as your insurer or benefits administrator.

When you use the Philips Sonicare App and Sonicare connected toothbrush, Philips will manage and process your personal data collected from your brushing activities. Please read the Philips privacy notice for more information.

Participant obligations. By participating in the BrushSmart Program, you, the Eligible Primary Enrollee agree to the following:

Ability to contract (legal age). The BrushSmart Program is offered to Eligible Primary Enrollees who are 18 years of age or older. By participating in the BrushSmart Program, you represent and warrant that you are of legal age to form a binding contract with Delta Dental and are the Primary Enrollee on your Delta Dental benefits plan.
