Delta Dental — Philips Oral Healthcare BrushSmartTM Program User Agreement
How the BrushSmart Program works:
- You, Eligible Primary Enrollees (defined as employees of the company aged 18 and over), will be offered an app-enabled Philips Sonicare DiamondClean Smart toothbrush at no cost to you ($229.99 MSRP) if you choose to participate in the BrushSmart Program (“Program”). This connected toothbrush will track your brushing habits.
- If you as a Primary Enrollee opt to participate in the Program, you will receive surveys to help Delta Dental of California, Delta Dental of Pennsylvania, Delta Dental Insurance Company, their corporate affiliates (“Delta Dental”) and Philips Oral Healthcare (“Philips”) understand your personal experience and satisfaction with the Program.
- Philips will share your brushing activity data with Delta Dental to align brushing habits with claims activity.
- Philips and Delta Dental will provide aggregate-level de-identified data to the Eligible Primary Enrollee’s Employer Group on brushing habits, satisfaction and the like.
Voluntary participation. Your participation is completely voluntary. You are free to choose not to participate, withdraw your consent or to discontinue to participate in the BrushSmart Program. Those actions will not result in any penalty or loss of benefits on your part.
Data collection and privacy policy. By participating in this Program you authorize Delta Dental to disclose to Philips that you have Delta Dental coverage and that these companies may transmit, analyze and manage your data. Philips will securely share the following data with Delta Dental via a secure FTP server using standard encryption and security measures to ensure your privacy and confidentiality: date, frequency of brushing, time stamp (AM/PM), duration, brushing mode and type of brush head used.
Delta Dental will provide aggregate-level de-identified data to the Employer Group on brushing habits, satisfaction and the like. Delta Dental’s Privacy Policy (available at: https://www.deltadentalins.com/about/privacy/) applies to all claims and enrollment information collected and maintained by Delta Dental as your insurer or benefits administrator.
When you use the Philips Sonicare App and Sonicare connected toothbrush, Philips will manage and process your personal data collected from your brushing activities. Please read the Philips privacy notice for more information.
Participant obligations. By participating in the BrushSmart Program, you, the Eligible Primary Enrollee agree to the following:
- You acknowledge that the BrushSmart Program is not available to all Delta Dental enrollees. You agree not to reference the BrushSmart Program name or details on social media channels (e.g., Facebook, Instagram, Snapchat, Twitter, etc.).
- You agree to be contacted via email during the registration process to ensure materials have been received and activated.
- In exchange for the Philips Sonicare DiamondClean Smart power toothbrush, you agree to:
- Be the sole and only user of the device provided as part of this BrushSmart Program.
- Download the Philips Sonicare application from the links provided in the BrushSmart Program materials to your personal phone or tablet.
- Sync the connected toothbrush and the Sonicare app on a weekly basis.
- Complete the surveys from Delta Dental and/or Philips as related to your usage and satisfaction with the BrushSmart Program.
- Allow your brushing and connected toothbrush usage data to be shared with Delta Dental.
- Provide your shipping information and telephone number for accurate shipping of the connected toothbrush to your home.
Ability to contract (legal age). The BrushSmart Program is offered to Eligible Primary Enrollees who are 18 years of age or older. By participating in the BrushSmart Program, you represent and warrant that you are of legal age to form a binding contract with Delta Dental and are the Primary Enrollee on your Delta Dental benefits plan.
