The new UC Berkeley Student Dental Plan began on August 15. Student enrollees will receive the maximum benefits when visiting a Delta Dental PPO dentist.
Plan Facts
- Group number is 04713.
- Use the enrollee's Student ID on claims. Do not use the social security number. (The student id is a "U" plus 8 digits.)
- Check a patient's eligibility and benefits in any of three convenient ways:
- Online. Log in and look up the information using our online services
- Phone. Call our toll-free automated telephone service 888-335-8227
- Electronic document. Download and review a PDF of the benefit highlights
- Electronic statements: Receive e-mail notifications when new documents are available. Sign up for electronic statements
- The plan follows a plan year, not calendar year, for administering benefits, maximums and deductible amounts. For example, August 15, 2010 through August 14, 2011.
